Lesson 1 NCLEX-RN Exam Overview and Safe, Effective Care
The NCLEX-RN is a computerized adaptive test (CAT) that selects each question based on your previous answers, so the difficulty rises when you answer correctly and falls when you answer incorrectly. Candidates receive a minimum of 75 questions and a maximum of 145 questions, including 15 unscored pretest items, with up to five hours to complete the exam. The test ends when the computer is 95% confident in your ability estimate, when you answer the maximum number of questions, or when time expires.
Every question is organized under one of four client needs categories: Safe and Effective Care Environment, Health Promotion and Maintenance, Psychosocial Integrity, and Physiological Integrity. Safe and Effective Care includes management of care, such as advocacy, delegation, and informed consent, plus safety and infection control. Physiological Integrity is the largest area and covers basic care and comfort, pharmacological and parenteral therapies, reduction of risk potential, and physiological adaptation.
The nursing process provides a systematic framework for clinical decisions: assessment, diagnosis, planning, implementation, and evaluation (ADPIE). Assessment is the first and most essential step; you collect subjective reports and objective findings, including vital signs and laboratory values, before you interpret them. After formulating a nursing diagnosis and measurable outcomes, you implement interventions and then evaluate whether the client response matches the expected outcome. This cycle repeats whenever the client condition changes.
Prioritization questions reward a consistent method. Apply the ABC framework (airway, breathing, circulation) first, then Maslow hierarchy, which places physiological needs above safety, love and belonging, esteem, and self-actualization. Acute problems generally outrank chronic ones, and unstable findings such as a falling oxygen saturation or a new dysrhythmia take priority over stable complaints. When assigning or caring for several clients, see the client with the most urgent, life-threatening risk first.
Safety is the foundation of nursing care. Use two approved identifiers, such as name and date of birth, before administering medication, blood, or treatments; a room number is never an acceptable identifier. Perform hand hygiene before and after every client contact, and use standard precautions with all clients. Contact precautions require gloves and a gown, droplet precautions require a surgical mask, and airborne precautions require an N95 respirator or PAPR in a negative-pressure room for conditions such as tuberculosis, measles, or varicella.
Delegation improves efficiency but never transfers accountability. The registered nurse must perform assessment, nursing judgment, and evaluation, and must verify the five rights of delegation: right task, right circumstance, right person, right direction and communication, and right supervision and evaluation. The LPN/LVN can carry out stable, predictable tasks such as medication administration, simple wound care, and reinforcing teaching. The unlicensed assistive personnel (UAP) can perform routine, noninvasive care such as bathing, ambulating, measuring vital signs, and feeding a stable client.

Example
Scenario: A nurse on a medical-surgical unit receives shift report. Client A is a stable postoperative client waiting for discharge teaching. Client B has type 2 diabetes and blood glucose of 72 mg/dL with no symptoms. Client C has asthma, new wheezing, and oxygen saturation of 88% on room air. Client D is comfortable and sleeping after receiving pain medication. Which client should the nurse assess first?
Correct action: The nurse should assess Client C first because the client has new wheezing with an oxygen saturation of 88%, which signals impaired oxygenation and risk of respiratory failure.
Why: The ABC framework makes airway and breathing the highest priorities. Client C is unstable and could deteriorate rapidly, whereas the other clients are stable or have findings that can be rechecked within a few minutes.
Worked example: Answer each question as if it counts, because the next question depends on the current one.
Lesson 2 Health Promotion and Psychosocial Integrity
Health promotion begins with prevention across the lifespan. Follow the recommended immunization schedule, which includes hepatitis B, rotavirus, DTaP, Hib, pneumococcal, and influenza vaccines in childhood, plus boosters such as Tdap, meningococcal, and HPV for adolescents. Adults need annual influenza vaccine, periodic tetanus boosters, pneumococcal and shingles vaccines when indicated, and a COVID-19 vaccine according to current recommendations. Screenings include blood pressure checks, lipid panels, blood glucose, mammography, cervical cytology, colorectal cancer screening, bone density testing, and vision or hearing examinations at the recommended intervals.
Growth and development questions test milestones and the psychosocial tasks of each stage. According to Erikson, infancy develops trust versus mistrust, toddlers learn autonomy versus shame and doubt, preschoolers develop initiative versus guilt, school-age children build industry versus inferiority, and adolescents search for identity versus role confusion. Piaget describes sensorimotor, preoperational, concrete operational, and formal operational thinking, while gross motor milestones progress from rolling to sitting, crawling, standing, and walking. Compare the client behavior with age expectations and plan teaching around the client developmental level.
Therapeutic communication is a core psychosocial skill. Use open-ended questions, active listening, silence, reflection, restating, and clarifying statements so the client feels heard and safe. Avoid false reassurance, closed questions that demand a simple yes or no, asking why, changing the subject, giving personal opinions, or offering unsupported advice. Sit at the client level, maintain eye contact when culturally appropriate, and allow the client to set the pace of the conversation.
Anxiety ranges from mild to panic, and the intervention changes with the level. For a client with severe anxiety or panic, reduce stimuli, remain calm, use short, simple sentences, and stay with the client. Depression is characterized by depressed mood, anhedonia, sleep or appetite changes, poor concentration, low energy, and feelings of worthlessness. Screen every depressed client for suicidal ideation using direct questions, and document the client plan, means, and intent. Teach that antidepressants may take 2 to 4 weeks to work and should not be stopped abruptly; monitor closely for worsening mood, especially in young adults.
Crisis intervention is time limited and action oriented: ensure safety, identify the precipitating event, mobilize support, and help the client use effective coping. Grief may follow loss through death, divorce, disability, or job loss, and clients may move through denial, anger, bargaining, depression, and acceptance in their own order. Respect cultural, religious, and spiritual beliefs about illness, treatment, diet, and end-of-life care, and use a trained medical interpreter rather than a family member when a language barrier exists. Finally, assess readiness to learn; pain, anxiety, low literacy, or language differences can block learning, so use teach-back, short sessions, and printed materials, then verify understanding.

Example
Scenario: A client whose spouse died two weeks ago says, "I keep crying, I cannot eat, and sometimes I just want to go to sleep and never wake up." The client denies having a specific plan. What is the nurse best action?
Correct action: The nurse should directly assess suicide risk by asking whether the client has thoughts of harming themselves, then evaluate intent, plan, means, and timeframe, keep the client safe, notify the provider, and involve mental health support.
Why: Expressions such as "never wake up" can indicate passive suicidal ideation and require immediate evaluation even when no plan is stated. Direct, nonjudgmental questions do not plant the idea; they provide the data needed for safety planning and further treatment.
Worked example: Airway problems come before pain management in priority questions.
Lesson 3 Physiological Integrity: Systems and Perioperative Care
Accurate vital signs begin with correct technique and known reference ranges. Normal adult temperature is about 36.0 to 37.5 C (97 to 99.5 F), pulse 60 to 100 beats per minute, respirations 12 to 20 per minute, blood pressure below 120/80 mm Hg, and oxygen saturation 95% to 100% on room air. Measure temperature by the route ordered or least invasive, count respirations for a full minute when the pattern is irregular, and use the correct cuff size because an incorrect cuff distorts blood pressure readings.
Fluid and electrolyte balance depends on intake, output, renal function, and hormonal regulation. Know normal values: sodium 135 to 145 mEq/L, potassium 3.5 to 5.0 mEq/L, calcium 8.5 to 10.5 mg/dL, and magnesium 1.3 to 2.1 mEq/L. Hypokalemia can cause weakness and U waves, while hyperkalemia can produce peaked T waves and cardiac arrest; hold potassium and notify the provider when the level is high. Monitor daily weights, strict intake and output, skin turgor, edema, and urine output, and identify clients at risk for overload such as those with heart failure or renal disease.
Cardiovascular assessment includes chest pain description using PQRST (provocation, quality, region, severity, timing), pulses, heart sounds, edema, and jugular venous distention. Chest pain with diaphoresis, nausea, or dyspnea suggests acute coronary syndrome and requires immediate assessment, 12-lead ECG, troponin, and oxygen if saturation is low. Respiratory assessment includes rate, rhythm, depth, work of breathing, breath sounds, and the use of accessory muscles; crackles may indicate pulmonary edema, while absent breath sounds may indicate pneumothorax or a mucus plug. Teach clients with asthma or COPD to use prescribed inhalers correctly and to report worsening symptoms early.
Renal assessment focuses on urine output, color, clarity, and laboratory values. Output below 30 mL/hour, a rising creatinine, or new edema signals impaired renal function and requires notification of the provider. Prevent catheter-associated infection by using the smallest appropriate catheter, keeping the drainage bag below the bladder, and maintaining a closed system. Gastrointestinal care includes assessing bowel sounds in all four quadrants, abdominal distention, nausea, vomiting, and elimination patterns, plus providing ostomy care, nasogastric tube management, and nutrition tailored to the client condition.
Neurological assessment evaluates level of consciousness, pupils, motor and sensory function, speech, and the Glasgow Coma Scale. Use the FAST acronym (face, arms, speech, time) to recognize stroke, keep the client NPO until swallowing is evaluated, and maintain seizure precautions. Musculoskeletal care includes fall and pressure injury prevention, neurovascular checks of the six Ps (pain, pallor, pulselessness, paresthesia, paralysis, poikilothermia) after casting or surgery, and interventions to prevent venous thromboembolism such as early mobility, sequential compression devices, and anticoagulant therapy.
Wound assessment includes location, size, color, drainage, odor, and surrounding tissue; document wound bed tissue such as granulation, slough, or eschar, and report redness, warmth, swelling, or purulent drainage as signs of infection. Use sterile technique for open wounds, irrigate gently, and choose the dressing that keeps the wound bed moist while protecting the surrounding skin. Perioperative care begins before surgery with NPO status, informed consent, allergy history, and baseline assessment, and continues after surgery with airway monitoring, pain control, early ambulation, incentive spirometry, incision inspection, and prevention of hemorrhage, infection, DVT, and urinary retention.

Example
Scenario: On postoperative day 2, a client who had abdominal surgery calls the nurse reporting sudden shortness of breath and chest pain. The client appears anxious, heart rate is 122, respirations 28, blood pressure 108/66, and oxygen saturation 90% on room air. What is the nurse priority action?
Correct action: The nurse should stay with the client, raise the head of the bed, apply oxygen, and immediately notify the provider while preparing for further assessment; sudden dyspnea, tachycardia, and hypoxemia after surgery suggest a pulmonary embolism, which is a life-threatening emergency.
Why: Airway, breathing, and circulation come first. The nurse must not leave the client or wait for the next vital sign check. Oxygen and provider notification are urgent because a pulmonary embolism can progress rapidly to hemodynamic collapse, and treatment such as anticoagulation or thrombolysis requires early recognition.
Worked example: A falling SpO2 with confusion requires immediate assessment and oxygen.
Lesson 4 Pharmacology and Nursing Fundamentals
Safe medication administration begins with the six rights: right client, right medication, right dose, right route, right time, and right documentation. The right reason, right response, and the client's right to refuse are also considered whenever appropriate. Before giving any dose, compare the original order with the medication administration record and verify the client using at least two identifiers such as full name and date of birth. Never prepare a medication from memory or from another client's supply.
Drug calculations use the basic formula D/H x Q: the ordered dose divided by the dose on hand, multiplied by the quantity or volume. Weight-based doses are calculated in mg/kg and checked against a safe range before administration. Use metric units, avoid trailing zeros, and place a leading zero before a decimal point when the dose is less than one. If the result looks unusual, recalculate and ask a second nurse to verify before giving the medication.
Insulin requires careful timing and technique. Rapid-acting lispro and aspart begin working in about 15 minutes, peak in about 1 hour, and last 3 to 5 hours. Regular insulin begins in 30 minutes, peaks in 2 to 4 hours, and lasts 6 to 8 hours. NPH peaks in 4 to 12 hours, while long-acting glargine or detemir provides a relatively steady 24-hour effect. Use the correct insulin syringe, rotate injection sites, give the right insulin at the right meal time, and never share pens or needles.
Opioid safety includes monitoring sedation, respiratory rate, and oxygen saturation. Naloxone reverses opioid-induced respiratory depression within minutes, but it may be shorter acting than the opioid, so repeat doses and continued monitoring are essential. Anticoagulants also require vigilance: warfarin is monitored with INR, heparin with aPTT, and enoxaparin usually needs no routine laboratory monitoring. Hold digoxin for a pulse below 60 or signs of toxicity, and hold metformin before iodinated contrast procedures when renal function is impaired.
Antibiotic stewardship means watching for allergic reactions, nephrotoxicity, ototoxicity, tendon pain, Clostridioides difficile diarrhea, and red man syndrome. Nursing fundamentals support every clinical decision: document promptly and objectively, never alter records, turn and reposition clients every two hours, use gait belts and fall precautions, provide perineal care from front to back, and assist with ADLs while encouraging as much independence as possible.
Client education is part of medication safety. Use plain language, teach-back, qualified interpreters for language barriers, written materials at the client's reading level, and family involvement when the client agrees. Confirm understanding, the expected response, and the action to take if symptoms or side effects occur.

Example
A client prescribed digoxin 0.125 mg orally once daily has an apical pulse of 52 beats per minute, a potassium level of 3.1 mEq/L, and reports nausea. The nurse should withhold the digoxin, notify the provider, and prepare to obtain a digoxin level and electrocardiogram.
The correct action is to withhold the dose because bradycardia, hypokalemia, nausea, and possible visual changes are signs of digoxin toxicity. Giving the dose could increase the risk of serious dysrhythmias. The nurse should document the assessment, monitor the client, and be ready to administer treatment ordered by the provider.
Worked example: Before giving an antibiotic, check the client's allergy history and the prescription.
Lesson 5 Maternity, Pediatrics and Final Review Plan
Prenatal care begins with an initial assessment and continues through regular visits that track maternal blood pressure, weight, uterine growth, fetal position, and laboratory results. Folic acid 400 to 800 mcg daily reduces neural tube defect risk, and a balanced diet, prenatal vitamins, safe exercise, and routine screening support a healthy pregnancy. After about 28 weeks, teach the client to monitor fetal movement and to report danger signs: vaginal bleeding, severe headache, visual changes, epigastric or right upper quadrant pain, decreased fetal movement, leaking fluid, contractions before term, or fever.
Labor has four stages. The first stage includes latent, active, and transition phases and ends with full cervical dilation. The second stage extends from full dilation to birth of the newborn, the third stage is delivery of the placenta, and the fourth stage is the first hours of recovery. During labor, support positioning, hydration, comfort measures, and continuous assessment of contractions and fetal heart rate. Document cervical change, membrane status, maternal vital signs, and the client's response.
Fetal heart rate assessment includes baseline rate, variability, accelerations, and decelerations. The normal baseline is 110 to 160 beats per minute with moderate variability. Early decelerations are benign responses to head compression; late decelerations suggest uteroplacental insufficiency; variable decelerations suggest cord compression. For nonreassuring patterns, reposition the client on the left side, give oxygen, increase IV fluids, stop oxytocin, notify the provider, and prepare for possible cesarean or assisted birth while continuing to document.
Postpartum care includes checking the fundus for firmness and position, lochia color and amount, vital signs, voiding, pain, breasts, and perineal healing. Lochia normally progresses from rubra to serosa to alba. Teach perineal hygiene, handwashing, and how to use cold packs and sitz baths. Report hemorrhage such as more than one saturated pad per hour, fever, severe pain, calf swelling or pain, and mood changes suggesting postpartum depression.
Newborn assessment includes Apgar scores at 1 and 5 minutes, vital signs, weight, length, head circumference, and reflexes. Encourage breastfeeding within the first hour, check latch and swallowing, and teach feeding cues. A well-fed newborn has 6 or more wet diapers and several stools per day after the first days, returns to birth weight by about 2 weeks, and shows normal alertness and tone. Observe for jaundice, hypoglycemia, and respiratory distress, and reinforce safe sleep: back to sleep, firm mattress, and no loose bedding.
Pediatric care includes growth percentiles, developmental milestones, immunizations, croup, RSV, dehydration, and weight-based medication dosing. Follow the recommended immunization schedule for HepB, DTaP, IPV, Hib, PCV, RV, MMR, varicella, influenza, and HPV when due. For croup, keep the child calm and provide cool mist; for RSV, maintain hydration, suction the nose, and use droplet precautions when indicated. Mild to moderate dehydration is managed with oral rehydration, while severe dehydration requires IV fluids. Final review should focus on weak areas with practice questions, spaced review, rationales, and a realistic test-day plan that includes sleep, hydration, and timed practice.

- Link each stage of labor to nursing priorities and fetal heart rate patterns.
- Apply postpartum and newborn safety priorities, including breastfeeding and safe sleep.

Example
A 9-month-old infant weighing 8 kg has diarrhea, a sunken anterior fontanelle, dry mucous membranes, and no tears. The provider orders a normal saline IV bolus of 20 mL/kg. How many milliliters should the nurse prepare?
The correct action is to calculate 160 mL, verify the calculation with a second nurse, set the infusion pump to the prescribed rate, and reassess after administration. The weight-based dose is 20 mL/kg x 8 kg = 160 mL. These findings suggest moderate dehydration, and accurate pediatric dosing prevents fluid overload or under-resuscitation. The nurse should also monitor urine output, electrolytes, vital signs, and response to treatment.
Worked example: Report vaginal bleeding in the third trimester immediately.
